I am a little confused on the rule of do not use code Y92.9 if the place is not stated or is not applicable. I’ve had a number of charts that have no place of occurrence given on where the injury occurred.  What would you suggest we use in these instances?

That is an odd guideline, right, because the code itself is the unspecified location. What ICD-10 is looking for, is that someone has documented, place of injury unknown (or unspecified), and then you would assign the code. ICD-10 does not usually require these codes, and they don’t prefer unspecified codes, they want you to query providers (or patients) to find out. If you know, for instance, that it occurred in a house (but you don’t know if it is their house or a friend’s, neighbor, etc.), use an unspecified private residence code Y92.009 Unspecified place in unspecified non-institutional (private) residence as the place of occurrence of the external cause. Home (NOS) as the place of occurrence of the external cause. If you know it was a car accident on a street, use an unspecified street code, Y92.410 Unspecified Street and highway as the place of occurrence of the external cause. For an unspecified public place (found on ground, for example), use the Other specified place, Y92.89 Other specified places as the place of occurrence of the external cause. If you can’t get any info whatsoever, I would ask for documentation, place of injury unknown. Because Y92.9 is the only unspecified code, and as we do require this field for trauma registry, we need a code entered.
